Membership Resignation Form
Please use this form to cancel your Center membership. Notice of termination of membership must be submitted in writing thirty (30) days in advance. Additionally:
1) Any and all indebtedness has been paid in full or an acceptable payment schedule between the member and the Center has been agreed upon in writing, and
2) All items have been removed from any rented locker(s).
